Per indictments I know he was named as a Capo as early as 2004. This was something to do with a guy named Carl Carrara who ran Locals 14 and 15 for the Genoveses, but ended up flipping. It was a big indictment for a Museum of Modern Arts building that was being constructed worth around $400M. Muscarella got indicted with Louis Moscatiello and a bunch of other guys. He got five years in that case.
He was also listed as acting street boss in 2002 per here:

He was involved in the Orex mining pump and dump stock fraud scheme in 2006. Somehow he was named by the SEC but was pinched for it by the feds with Cafaro.

If he was acting boss by 2002 I wonder when he was made. Him being an associate of Buster Ardito would place him as a Bronx guy eh? Although I think Ardito wasn't 116th crew-wise right?
