Let's see here. They go from approximately 60 members to around half of that in 30 years. About a third of the remaining members are in their 80s or older. But they're going to last another 50 years? Grow a brain.

A relatively younger membership is one thing Philadelphia has going for it. And the fact they have continued to regularly make new members (15-17 new inductees from 2001-2019, according to Anastasia). They lost more than that during that time period but it does slow down the attrition.

Who does Scott say are the new Genovese Leaders in Springfield? What a load of bullshit that there's anything left at all to kick up from to New York. Everything there is gone. The restaurants, the strip clubs, the social clubs. I'm not going to pay for Scott's fiction but I am interested in what he has to say about Springfield. Does he have Albert successfully getting an envelope from the Manzis? My point is that the mob culture that produced the envelopes for NY in the early 2000's is completely gone and it's a generation later with the last leaders being the rat who lives there now and Calvanese, who's apparently still on drugs.

Medford has been mostly gone for a decade. There is the remnants of a sportsbook here north of the city and that's it. Maybe some low-level drug crimes and nothing else, which equals no indictments or a budget worth funding against any remaining Italians. It's pretty much over otherwise here. and Statistical Atlas has Medford at 22.5% Italian as of 2018

I think there's remnants of the family left for sure. But no future and I don't think there are any names floating around of younger guys in their 30's unless you're thinking of the youngest DiNunzio or Angelesca's kid. Other "younger" guys that get mentioned from time to time are basically legit union guys like Federico, who makes a great living now backing trucks into parking spots (he doesn't drive the trucks he just comes out and motions for them to back up.)
The other younger guys disputed on here for a decade like Marino and Scarpelli and Costa, even Angelesca are all ghosts and not leading any resurgence. And there are no other younger names to dispute. The guys who owned the businesses are dead or too old and their kids don't play ball. There are some old guys betting still and definitely a few younger guys betting but with all the sportsbooks available here now its hard to find kids to bet underground. It's just the way it went.
