When Scarfo was trying to rally for support in becoming boss, he spoke to capo Santo Idone who told him he would back him only if he (Scarfo) agreed to get along better with his uncles. I believe Scarfo eventually making Nick Piccolo consigliere was him honoring his word despite his "hatred". I don't know where the problem originated between Scarfo and the Piccolos, but it may coincide somehow with Scarfo going under Skinny DiTullio's wing.
