Maybe Billy Russo due to his fathers time in various leadership positions and because he's not addicted to jail like Teddy? No idea if he's as respected as his father though.

Cacace is 82. It would make much more sense to have him as Boss for a while, allow Teddy to get off restrictions and take the time to learn how the family are currently operating and who everyone is without as much heat on him before taking over in a few years. The Colombo's or the Mafia in general of course aren't about what is the best course of action when it comes to power though.

Andrew Russo was a cousin of Carmine Persico.So yes it count because many wiseguys in the family are related by blood to Persicos.
