There may be some guys doing some mid level stuff but almost all the heroin is coming in thru Mexico and they’ve got their own ppl in the states to handle the trafficking. Honestly the number of “connected” guys using heroin is probably much greater than the number of guys moving it.
And while Hootie Russo was certainly known to embellish a little bit, he said in his experience the Mexicans wanted absolutely nothing to do with the Italians. The Mexicans felt the Italians had too much Fed heat on them and it would only bring trouble. This was his “experience” like 25 years ago—who knows if there’s any truth to it.

The modern-day LCN is more involved in marijuana, prescription drugs and, to a lesser extent, cocaine. Their involvement in other drugs like heroin, meth, etc. is pretty negligible. You can see this in mob drug cases over the last 20+ years.

I think one big factor in guys in the life not really being into heroin seems to be the sentences handed down because of it
Matthew Madonna and Tom Mix Santoro got hit with 20 years, early on in their careers because of it.
An aforementioned reason about most of the heroin coming through Mexico is one of the other likely reasons

This is the central factor IMO. The networks of international heroin trafficking wouldn’t give any space for the mafia to have any major, high-level trafficking role to any significant degree today. Sure, mob guys don’t want the heat and sentences associated with big narcotics cases. But, even if they were willing to take those risks, I just don’t see a role anywhere approaching Pizza Connection-tier operations as possible for them today. Besides, most of what’s called “heroin” today is just stepped-on fentanyl, essentially. This is a China-to-Mexico-to-US pipeline. Since the Sicilian faction of the Gambino’s have taken over, I’ve read that it has allegedly increased their interest in heroin trafficking but recent indictments against the Gambino Family do not mention heroin

The Pizza Connection was a partnership between the Sicialians and the Americans but now the big player in drug trade is the ndrangheta that dont need the help of what remain ofcthe american mafia so the american wiseguys sell pills,weed or coke.
Matt Guglielmetti was linked to Dino Guilmette that was arrested for selling pills in Rhode Island.

I think the ndrnghets sell there drugs to Europe more then the us
Studies shows now that Europe is now the cocaine capital- not the USA like in the past.
That being said u have two major suppliers circumstances also majorly change from times like in the 60s/70s/80s and now
Afghanistan / golden triangle is not what it was when they were the lead heroin suppliers

1. The Americans had an active ban....from the Top, the Commision. Yet it's often presented like they were equal, ACTIVE partners... How do you reconcile that? which brings me to point 2...
2. The emphasis is always... " The Americans don't need drugs". Until you suggest the Sicilians managed the trade... then the Americans suddenly become partners, even as the rank and file are actively banned under punishment of death.

This is precisely WHY I found BOTH Old and New Bridge so intriguing. In Old Bridge, the Sicilians DID need the Inzerillos contacts, and in New Bridge, the Ursino also accessed Gambino South American contacts. That was particularly interesting, as there are NUMEROUS Calabrian clans embedded in South America, so why not go through any one of them?
I find it seems, when you talk about Sicily or Calabria, some guys hear, " Fuck the Americans"....then the convo goes left....
