As for why Milwaukee ended up answering directly to Chicago, here's Maniaci's explanation:
Milwaukee Crime Family boss Sam Ferrera tried to take over a gambling joint operated by Balistrieri. Balistrieri objected so Ferrera shelved him and may have even plotted to kill him. Balistrieri's father-in-law, capo John Alioto, reached out to Chicago for help. Accardo and Giancana decided Ferrera acted improperly, replaced him with Alioto and reinstated Balistrieri. Chicago never left.
