There are many sources over the years that potentially indicate that in terms of the actual chain of command, the outfit was a bit unique, and that consigliere, or chairman of the board, may have actually been the top of the food chain. What we would refer to as “the boss” still had autonomy to run the organization as needed, but likely had to get approval on key things (making guys, killing made

Sarno was obviously appointed to take the reins of the Outfit by the admin. This probably had to do with his age and his eagerness to make money. He was probably made in the late 90s after getting out of prison. A lot of people were surprised that he was in the boss position bc of his age. But no matter what is said about his old mugshots and how he looked like a biker, all the people still around only have good things to say about him and hope that he gets released bc of him being a standup guy.
