Except they were dismissed by the boss or even killed on the Bosses orders. Assuming he was even dismissed. He could have voluntarily stepped down for whatever reason.
I believe so but in reality the Boss can do what he wants.

Well if you consider Mario to be a successful capo, why not his father Giuseppe who served as a highly respected elder advisor/capo for 50 years? Although he may have served as UnderBoss at some point, not sure.

Traina was consigliere in the 1920s and acting capo dei capi for D'Aquila, then served on the peace committee in 1931. Also served as an aide for the Commission for close to 40 years. By far one of the most overlooked and influential figures in mafia history.
