Merlino was the shooter in the attemped murder or Nicky Scarfo Jr, he supervised the attempted murders of Joey Ciancaglini Jr and Joey Stanfa, and he was the getaway driver in the attempted murder of Anthony Milicia (by which point he was the official underboss of the family).

During a sitdown, is it always necessary to have a higher-ranking member in the room to act as a mediator? For example, if two soldiers have a dispute, can they get together on their own and try to resolve it, or do they have to call in the consigliere?

Also, which members are allowed to act as mediators? Considering how many disputes occur on a day-to-day basis, I doubt the consigliere could handle every single one.

From my understanding of mob protocol, the sitdown is a formal procedure that's invoked when the parties can't settle the dispute themselves. They request a higher-ranking member to decide for them. It's more of a final ruling rather than mediation. If soldiers have a disagreement, they go to the captain; if captains have a disagreement, they go to an admin member; if bosses have a disagreement, they go to the commission.

What if two soldiers from different crews have a dispute?

Each one of them goes to their respective captain. The captains meet up, and if they can't reach an agreement, they take it further up. Sitdowns were a big deal, despite being fairly common. The ability to present your arguments in a convincing way was a valued trait.

Something that would map out the relationships between the Gambinos, Castellanos, Inzerillos, etc. It can get quite confusing trying to figure out who's who when reading about that family since there are so many Giuseppes, Tommasos, Joes and Tommys, etc. Am I also correct in believing they're connected to the Sicilian element in Montreal through Paul Castellano's wife being a Manno?
This tree came to mind (from Salvatore Lupo's "The Two Mafias"), though it's not going to help with the Montreal connection.

Has there been any mobsters apart from Sonny Black that knew that they will die if they went to a certain meeting or met someone.

Bonanno Associate Louis Tuzzio. When called to a meeting he told someone (forgot who but it might have been his mom) that he was either going to get made or he was going to be killed. He was killed.
