Yes, would need the approval of both bosses. One time the former boss died and his successor wasn't informed, so he didn't accept the transfer. That was Jimmy Fratianno. His former boss, Frank Desimone, didn't tell his underboss, Nick Licata, that he transferred to Chicago. When Licata became boss he asked Chicago capo Frank LaPorte and he didn't know anything about it. Sam Giancana was in Mexico and unavailable, and he apparently didn't tell anyone that he accepted Fratianno. He was made into the Gambinos in the late 90s/early 2000s from what JD discovered.
I checked the book and it looks like Garcia was loose with his use of the descriptive word of member. He probably meant that he transferred from the Lucchese family to the Gambinos as an Associate.
