Cleveland vs. Pittsburgh fighting over Youngstown area circa 1980, about 8~10 people were killed (a couple of the murders you see on the list were actually internal housecleaning by the Naples/Prato organization that happened to coincide with the war, hence the indeterminate victim number).

Also the bombing etc. wars over the same region in the late 50s/early 60s, though the various parties involved are hard to figure out and it's kind of too chaotic to think of it as a war with two or three defined sides rather than just general mayhem. But it had the same cause: struggle for hegemony. The "Beer Wars" in Chicago in the 20s and early 30s also fit this description -- the mafia was involved, but it was just general mayhem rather than a bilateral war.

Scarfo vs. Riccobenes was another binary/bilateral conflict in the early 80.

Also some of the experts here might be able to elucidate how some of the things that looked like they were part of chaotic multilateral "Beer Wars" in Chicago in the 1920s were actually binary/bilateral mafia (using the term "mafia" precisely rather than broadly) wars between two opposed factions, but that's outside my wheelhouse.

Speaking of Chicago, there was something called the "Cheese War" which was apparently exactly what it says on the label :lol:, but I don't know much about it.
