Rizzo was an old timer from Palermo born in the late 1880s who could have been an early member of the D'Aquila family. He was definitely a captain for some period and I doubt he would have been promoted in the later 1950s given his advanced age at the time. My understanding, and JD or HK can correct me if I'm wrong, is that Rizzo ran a large crew centered in Brooklyn and that several other Brooklyn crews may have split off from this, or a couple members from his crew were promoted to captains of other crews. Again, I could be wrong, but I was under the impression that Rava split off from the Rizzo crew, probably after Anastasia became boss. Don't take any of this as fact, though.
Rizzo's son and grandson, both named John, are of course well-known members, talked about by Gravano and DiLeonardo quite a bit.
