This is "Flipper" Smaldone, he was the son of Eugene "Checkers", the guy who eventually became the family's boss towards the end.
It also seems unlikely that he was inducted, given the description of him in the Smaldone book:

Any idea if he is still alive? Probably shunned by Clyde’s family but not his immediate family. Clyde was actually shunned by his brothers. Supposedly he became an evangelical later in life. There is a possibility he was never made because after he got out of prison in the 60s he had no or little involvement in the criminal organization which shows when he was never listed on any hierarchy list in the 70s and 80s. There is a big possibility that the Smaldone were made sometime in the late 60s.
It is my understanding that all the elder Smaldone brothers, with the exception of Anthony, the restaurateur, were members of the Colorado family. Clyde would have been ~ 56 when he got out of prison and stepped away from organised crime, so I personally don't doubt that he was a member by then - he is carried as one by the FBI in both 1985 and 1993 either way.
From the Smaldone book, on Eugene "Flipper" Jnr:
Checkers was a career criminal, but he loathed drugs and drug dealers. Eventually, he and the rest of the family shunned Young Eugene. The final break came when he told his son, “You get out of my life. I don’t want any part of you. You’re nothing but a dopehead.”
I think a total falling out like this would eliminate any chance of induction, since towards the end of the 1970s (Pueblo leaders Colletti and Spinuzzi were dead by 1975), whatever was left of the powerbase had seemingly shifted to the Denver area and the active Smaldone brothers would have been the ones making membership decisions - at least in theory.
While the interaction is not dated, Young Eugene/Flipper probably had fallen out with his personal family (who pretty much made up the entire LCN one) earlier than the time of his drug case in 1973 and it seems unlikely that he would have been a member for that reason. That is my speculation, at least.

To answer this, Gallo talks about in Breakshot and on his blog, Villano dealt with LA caporegime Luigi Gelfuso over some sort of dispute related to a Las Vegas hotel that both families had an interest in. Gallo met Villano and his nephew (unnamed but since Paulie was already a nephew of the Smaldone family, I doubt it was either of the Eugenes) with Jimmy Caci, Stephen Cino and Gelfuso, who all indicated that he was the "same" as Milano.
